Responsibilities

Empower Individuals to Succeed
- Provide case management, vocational rehabilitation, and individualized support services to adults with traumatic brain injuries.
- Assist participants in developing and implementing individualized service plans focused on vocational and personal goals.
- Support skill development, independence, workplace readiness, and community integration.
Collaborate with a Dedicated Team
- Work closely with a multidisciplinary team, community partners, employers, and support networks to ensure successful outcomes.
- Foster a positive, supportive environment that promotes participant safety, growth, and productivity.
Drive Quality and Excellence
- Monitor participant progress and document outcomes through timely reports and case notes.
- Mentor consumers to develop strong work habits and maintain high standards of quality and performance.
- Contribute to continuous improvement efforts that enhance participant success and customer satisfaction.
- Supervise and support team members to ensure effective service delivery.

Requirements

Preferred
- Certified Occupational Therapy Assistant (COTA) credential.
- Experience working with individuals with traumatic brain injuries, neurological conditions, disabilities, or vocational rehabilitation programs.
Required
- Bachelor's degree in Human Services, Social Work, Rehabilitation, Psychology, Occupational Therapy Assistant, or a related field; equivalent experience may be considered.
- Strong written and verbal communication skills.
- Proficiency in Microsoft Office (Outlook, Word, Excel).
- Excellent organizational and case management skills.
- Ability to lift up to 20 pounds regularly.
- Ability to stand, stoop, bend, kneel, and squat as needed.

Salary

Pay Range: $23.08 - $25.00 per hour
