About the Position
Behavioral Health Patient Care Manager - PRN - Traverse City, MI
Responsibilities
The Patient Care Manager facilitates progression-of-care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, cost effective, and authorized by insurance companies.
Requirements
- Master’s degree from a social work, psychology, and/or counseling accredited program OR 6000 hours of counseling and/or related experience
- Michigan License to provide behavioral health counseling - May be a LMSW, LPC, or LLP With manager approval, Limited Licensed individuals may be considered.

D6 Inpatient Behavioral Health is a 17-bed unit dedicated to adult patients facing various mental health conditions, including schizophrenia, bipolar disorder, and depression. This unit boasts a multidisciplinary team comprising psychiatric providers, nurses, clinicians, and behavioral health technicians.
