Behavioral Health Patient Care Manager - Part Time Day Shift - Traverse City, MI
Munson Healthcare, northern Michigan’s largest healthcare system, is seeking a compassionate and skilled Behavioral Health Patient Care Manager to join our team in Traverse City, MI. This part-time day shift role offers a unique opportunity to make a meaningful impact in a community that values both work and lifestyle.
Responsibilities
- Facilitate progression-of-care and monitor patient progress to ensure high-quality, evidence-based care.
- Engage with patients across all age groups, assisting them in navigating complex emotional and physical challenges.
- Ensure the plan of care and services provided are patient-focused, efficient, cost-effective, and authorized by insurance companies.
Requirements
- Master’s degree from an accredited program in social work, psychology, or counseling OR 6000 hours of counseling and/or related experience.
- Michigan License to provide behavioral health counseling (LMSW, LPC, or LLP).
- Soft Skills: Empathy, Effective Communication, Team Collaboration.
Nice to Have
- Experience in a multidisciplinary team setting.
- Strong therapeutic communication skills.
