Behavioral Health Counselor II - Family Practice

At Munson Healthcare, we are seeking a dedicated Behavioral Health Counselor II to join our Family Practice team. If you are passionate about providing individualized treatment to patients and their families, this role is for you.

Responsibilities

• Provides individualized treatment to patients and their families through assessment, diagnosis, planning, education, intervention, discharge planning, and evaluation of outcomes.
• Conducts didactic and treatment group and individual sessions.
• Demonstrates competency through the annual competency assessment, customer evaluation, clinical staff dialogue, performance appraisals, and staff education.
• Documents application of the treatment process focusing on outcome criteria in accordance with hospital, departmental, and regulatory agency requirements.
• Communicates with other members of the health care team through case reviews to ensure continuity of care and coordination of services.

Requirements

• Master's degree and license in Social Work preferred with 3 years or 6000 hours of related work experience.
• Licensed in good standing in the State of Michigan as a Mental Health Professional.
• Minimum 2 years of mental health experience. Prefer experience with co-occurring disorders (mental health and substance use disorders).
• ICRC-certified or certifiable chemical dependency counselor and/or licensed preferred.
• Demonstrated knowledge and proficiency in the treatment of Mental Health Disorders.
• Demonstrated knowledge and proficiency in substance use dependency treatment concepts and applications with a focus on evidence-based practices.
• LLMSW - Limited License will also be considered.
