Senior Backend Engineer / FHIR & Integrations

About the Position

We are seeking an exceptional Senior Backend Engineer / FHIR & Integrations Specialist to become the foundational technical hire for a rapidly expanding Digital Health scale-up in Germany. Your core mandate is to own the integration domain, connecting a proprietary, battle-tested eHealth platform to the complex German healthcare infrastructure, including PVS systems, TI, and FHIR.

• With a Series A closing soon, you will join at the ground floor and help lay the technical foundation for a nationwide footprint of 100+ physical clinics and a hospital
• You will help build a European “Lighthouse” project setting standards for data governance, systems of record, and safe AI use in regulated healthcare
• There is a clear strategic trajectory towards a highly lucrative exit within the next 3 to 4 years

Responsibilities

• Take full ownership of the integration domain, building scalable connections between the core platform and German healthcare infrastructure
• Design and implement secure, DSGVO-native architectural solutions supporting the platform’s transition into a commercially licensable B2B SaaS
• Collaborate closely with the interim CTO and product leadership to manage the codebase, resolve technical challenges, and prepare the ground for a specialised EU-based engineering team

Requirements

• 5+ years of backend development experience with strong expertise in TypeScript and Node.js
• Solid hands-on experience with NestJS or comparable modular backend frameworks and PostgreSQL
• Proven experience integrating with German PVS systems such as Tomedo, CGM, or x.comfort
• Deep knowledge of healthcare data standards such as FHIR, HL7, or xDT/BDT formats
• Familiarity with KBV technical specifications, the Telematikinfrastruktur connector landscape, and DSGVO-compliant architecture
• A modern engineering mindset, high autonomy, and comfort with early-stage ambiguity
• Fluent English; German is a strong plus, and Spanish is also a plus
